# **Consumer Testimonial Form**

If you were pleased with the way we took care of your loan, please consider leaving us a testimonial.

By submitting this testimonial you hereby agree and confirm

1. GVC Mortgage, Inc. has the right to use your testimonial, which includes: (1) your personal testimonial/endorsement, whole or part, regarding products, service received, customer service, or any other appropriate comments, and (2) your first name, last initial, and city and state in which you live.
2. The testimonial may appear in connection with any type of advertisement, including but not limited to: electronic and print media, television and radio, and any web based advertising.
3. You are over the age of 18 years old.
4. You hereby release GVC Mortgage, Inc. from any and all claims and expenses arising from your testimonial.
